Make Anonymous Complaint

If you would like to make an anonymous complaint you may do so by sending a letter to 379 Whispering Pines Road, North Wilkesboro, NC 28659 or you can fill out the form on this page.

 

Be sure to add the following information to your complaint:

• Reason for the complaint

• The person(s) involved in the complaint

• Date of the incident

• Details of the incident

• Resolution suggestions

 

Anyone wishing to review our quality outcomes report/strategic plan/culture competence plan/or survey results may do so at either office location during normal business hours.
